As you know, we<\/span> routinely take care of <\/span>the most medically complex and surgically ill <\/span>patients <\/span>\u00a0– those with the highest case-mix index – <\/span>while <\/span>we deal with issues of capacity and resource needs. <\/span>As technology has advanced<\/span> and become more widespread<\/span>, community and regional\u00a0 hospitals <\/span>now care <\/span>for patients historically sent to us<\/span>. \u00a0This creates a higher level of acuity and complexity in<\/span> our <\/span>tertiary care <\/span>system. <\/span>\u00a0<\/span>In fact, t<\/span>he number of extremely compl<\/span>icated<\/span> patients in our hospital<\/span>,<\/span> at any given time<\/span>,<\/span> is at an all-time high and getting higher. <\/span>\u00a0Inpatient and outpatient volumes have also increased in the last two years.<\/span><\/span><\/p>\n

A seamless transition is needed for these complex patients during transfers. \u00a0Using Be Safe methods and the structure of the unit based leaders (UBL), a team which included Larry Gimple, Jamie Kennedy, Mark Adams and Joseph Schoeny, created standard work on information transfer for incoming CCU patients. \u00a0\u00a0<\/span><\/span><\/div>\n<\/div>\n

\u00a0<\/span><\/span><\/div>\n<\/div>\n

As a result of their work, wh<\/span>en a<\/span>n outside <\/span>physician requests a transfer<\/span>,<\/span> a cascade begins. <\/span>A checklist is used to ensure that CCU fellows and attendings have accurate information in \u201creal time\u201d while a <\/span>“Heart” HIS analyst <\/span>(<\/span>24<\/span>\/7)<\/span> review<\/span>s<\/span> and\u00a0confirm<\/span>s <\/span>that <\/span>the <\/span>relevant images have been uploaded successfully. <\/span>Our in-house Patient Placement Center organizes a conference call for our clinicians, <\/span>the referring providers<\/span>, and any other needed groups<\/span>.\u00a0 The acceptance and details of the <\/span>transfer <\/span>are\u00a0posted on an CCU BeSafe Board using a standard tool. <\/span>The CCU team is then able to know that they have everything they need <\/span>to be able to plan for the next level of care.\u00a0 These t<\/span>ransfers are logged, audited and reviewed by Mark Adams and the rest of the Be Safe team. <\/span><\/span><\/div>\n

Mike D. Williams’ new position as the Associate Chief Medical Officer for Clinical Integration<\/span> ties into transfers as well. Mike and his team f<\/span>acilitat<\/span>e both the <\/span>intr<\/span>a-<\/span>facility and inter-facility communication and\u00a0oversee<\/span>s<\/span> the Patient Placement Center, which incorporates the <\/span>new in-house Bed Center and Transfer Center. Having Mike in this role means that we have a clinician with deep understanding of our processes and our system and someone who can understand the needs of the patient as well as <\/span>what <\/span>resources <\/span>are <\/span>available. This <\/span>patient can be received in the <\/span>\u201cright\u201d bed with the \u201cright\u201d providers by their side<\/span> in the most efficient manner. <\/span>\u00a0<\/span><\/span><\/div>\n<\/div>\n
